The Diagnosis and Treatment of Recurrent and Chronic Sinusitis in Children

The incidence and history of chronic and recurrent sinusitis in children is not easily known or tracked, largely because there is no general agreement on which symptoms together indicate sinusitis. The disease is ever-changing, probably has multiple causes, and lacks one specific set of symptoms.

Subglottic Stenosis Complicating Cardiac Surgery in

Children

Children frequently need mechanical ventilation immediately following cardiac surgery. Though this step is critically important to determine how the surgery turns out, debilitating injury to the laryngotracheal system can result. Memphis otolaryngologist, Dr. Rande Lazar, uses his focus on pediatric ear, nose, and throat problems to discuss treatment of acquired laryngeal stenosis, a complication of pediatric cardiac surgery, resulting from these laryngotracheal injuries.

Peritonsillar Abscess in Early Childhood: Presentation

and Management

Children under five years old who exhibit poor oral intake, a high fever and drooling might have a peritonsillar abscess. Though tonsillectomy is a safe and effective method for draining such an abscess, and immediate treatment greatly reduces the effects of the problem, diagnosis must first be achieved, via a tomographic scan of the neck. Dr. Rande Lazar, an expert in pediatric ear, nose, and throat disorders, is familiar with the management of these abscesses.

Ventilation Tubes and Prophylactic

Antibiotic Eardrops

A study conducted by Rande Lazar MD and his colleagues has shown that preventative antibiotic eardrops in the ears of children having ventilation tubes inserted as a means of treating chronic otitis media does not significantly decrease the risk of complications including otorrhea. This article, hosted at the Memphis Ear Nose and Throat blog details the procedure and its results.

Outpatient Adenotonsillectomy: Is it Safe in Children

Younger than 3 Years?

Dr. Rande Lazar, a Memphis otolaryngologist with a primary focus in pediatric ear, nose, and throat disorders and their treatments, participated in a research study about outpatient adenotonsillectomy and the safety of the procedure in children younger than three years. While the procedure can safely be performed as an outpatient operation, and the patient sent home the same day, there are concerns about compromising the safety of patients under the age of three when they are discharged on the same day. In this paper at the Memphis Ear Nose and Throat blog, Dr Lazar and his colleagues investigate the safety of the procedure.

Endoscopic Drainage of Subperiosteal Abscess in

Children: A Pilot Study

Subperiosteal abscess is a serious complication of sinusitis predominantly affecting children. The traditional approach to draining this abscess is drainage through an external incision; however, in this study, it was treated with functional endonasal sinus surgery. Ten pediatric patients with clinical and CT evidence of the subperiosteal abscess were treated.

Despite days of treatment for their sinusitis, these patients’ symptoms were increasing, not abating. Tests pointed to subperiosteal abscesses. Once treated with endoscopic surgery, all patients showed significant improvement within just 12 to 24 hours. The patients were treated with antibiotics following surgery.

No complications or returning abscesses were found during a follow-up visit after 2 to 3 weeks. The study appears to confirm that endoscopic drainage of subperiosteal abscess offers the patient an alternative to traditional drainage methods – one that prevents an external scar.

Schwannomas of the Paranasal Sinuses

Schwann’s cell tumors, or schwannomas, arise from the neural sheath of several kinds of nerves. In this article by the Memphis ear nose and throat specialist Dr. Rande Lazar, a patient was treated for a malignant schwannoma through surgery, radiotherapy, and radon seed implants. After three years, the patient remains free of symptoms – showing the benefits of radiotherapy for Schwann’s cell tumors.

Granular Cell Tumor of the Larynx: Report of Two Pediatric Cases

An uncommon neoplasm of the head and neck, granular cell tumors affect more blacks than members of other racial groups, and they affect more females than males. However, while these tumors can involve the throat, that occurrence is more frequent in males than females. Rande Lazar, a Memphis ear, nose and throat specialist, has experience dealing with these tumors. In this paper at the Memphis Ear Nose and Throat blog, he discusses two cases of these tumors in children.

Children with chronic sinusitis

Because chronic sinusitis can be so difficult to pin down, managing the disease and determining the patient’s need for surgery requires evaluating the disease. X-ray films do miss many things that show up on computed tomography (CT) scans, including slight variations and anatomic structures. Rande Lazar, a Memphis ENT with a pediatric focus, did a study comparing CT, x-rays, and a few other scanning techniques for their effectiveness in detecting chronic sinusitis symptoms.

Though CT scans vastly outperformed all other scanning techniques, determinations about a diagnosis or possible surgery should still be made considering symptoms and physical exams along with the results of the CT scan.
